Viewpoint Screening
Viewpoint is a required online database service that stores and tracks all required health records in one place! This helps you access records at any time in the future should you need them for employment or continuing education.
1. Create your account! *Fee required.
2. Visit Viewpoint Screening
3. Click “Start Your Order”
4. Choose Package: Nursing or Social Work (Health Sciences program no longer enrolling new students)
5. Nursing students: You must select “Drug Test + Health Portal” when creating your account.
6. Complete Applicant Information and Payment
7. Log In and Submit documents!

Re-admitted students:
·Required to complete the following (as described below):
- Viewpoint 10-panel drug screen w/ marijuana
- Fingerprinting/records transfer
- DCFS background check
- TB testing: negative 2 step (4 visit) skin test or negative QuantiFERON blood titer (must be dated within six months prior to the first day of classes)

Health Records Required by the State of Illinois
All newly admitted and re-admitted students to Methodist College are required to provide proof of immunity of the following:
· Measles, Mumps, & Rubella (MMR)
- Two vaccine doses or a positive IGG titer for each component
- Tetanus, Diphtheria, and Pertussis (Tdap)
- Post 18th birthday
- Within the past ten years
- Renewed every ten years
· Meningococcal Conjugate
- Those born after 1995: Vaccine administered on or after 16th birthday
Health Records Required by Methodist College
The following are required by Methodist College and Carle Health:
· Hepatitis B (HBV)
- Three vaccine doses AND Quantitative Surface Antibody titer (Positive result is ≥ 10 IU/mL). If titer is negative or if it is positive but no proof of full series, contact Student Health Records for further guidance.
· Varicella
- Two vaccine doses or positive IGG titer
· Seasonal Influenza Vaccine
· Department of Child and Family Services (DCFS) Background Check
· Fingerprint Background Check
- Conducted at New Student Orientation
- Fee Required (prices subject to change):
- Fingerprinting (cash/check): $60
- Fingerprinting (credit/debit card): $65
- Records transfer (cash/check): $30
- Records transfer (credit/debit card): $35
· TB: Negative 2-Step (4-visit) tuberculin skin test or negative QuantiFERON blood titer
- Within six months prior to the first day of classes
- Must be renewed yearly with a negative 1-step skin test or negative QuantiFERON Gold Blood Test
· COVID-19: Highly recommended but not required
· Hepatitis B Confirmation & Declination Form: Must be completed by every student (please see Hep B Flowcart for more clarification).
Program-Specific Health Records Required:
The following are additional requirements per program. Health record requirements are subject to change due to state, federal, and clinical location requirements.
Nursing:
· 10-Panel Drug Screen with marijuana through Viewpoint Screening; Fee required.
· CPR: Not due until the student begins clinicals. Certification must be Basic Life Support for Healthcare Providers via the American Heart Association. Online courses will not be accepted unless they are accompanied by a hands-on portion (please see https://www.methodistcol.edu/cpr or speak with your clinical director for more information). RQI Program available on campus. (*Fee required)
Social Work:
· No additional program-specific requirements
Health Record Exemptions
Students can request an exemption from required immunization(s) or health record(s) for the following reasons:
· Exemption due to a medical condition
· Exemption due to religious beliefs
*For exemptions, contact StudentHealthRecords@methodistcol.edu.
For medical exemptions: The form must be completed and submitted with documentation from a healthcare provider verifying the immunization(s) cannot be administered due to health reasons.
For religious exemptions: The form must be completed and submitted with an electronically verified signature or must be signed by hand.
CLINICAL EXEMPTIONS: Many of our clinical partner organizations maintain their own health and immunization requirements and may not accept medical or religious exemptions. Because completion of clinical education experiences is a mandatory component of nursing and other healthcare programs, students who are unable to meet a clinical site's vaccination requirements will be ineligible to participate in required clinical placements and, therefore, unable to complete their program of study. Methodist College is unable to provide alternative clinical placements or accommodations that waive a clinical site's immunization requirements.
